A recent sonogram for unexplained left-side pain revealed that I have a 2.1 cm fibroid. My doctor says that it is "little." How big does a fibroid need to be for a doctor to consider removing it?

It is more of a question of whether you have symptoms or if interrupts your patterns of daily living than the actual size of the fibroid. If it is not bothering you, there is no reason to remove it. Invasive procedures are considered a last resort.
There are many different treatments for them some invasive some not. I will include a link for you to do some research on. You should be able to make and informed decision on which path is best for you in relation to your symptoms.
